1 Not All Acne Scars Are the Same
Understanding your scar type is essential because different scars require different treatments. Atrophic scars (indented): Ice pick scars are deep, narrow puncture-like scars. Boxcar scars have sharp edges and a broader base. Rolling scars have a wave-like, undulating surface. Hypertrophic and keloid scars are raised, thickened scars more common in darker skin tones. Post-inflammatory hyperpigmentation (PIH) is technically not a scar but a dark mark left after acne, which fades over time with the right treatment.
2 MNRF (Microneedling Radiofrequency)
MNRF is our most effective treatment for atrophic acne scars. It delivers radiofrequency energy through insulated microneedles directly into the dermis, triggering powerful collagen remodelling deep within the scar tissue. Unlike ablative lasers, MNRF works equally well on all skin tones including Indian skin. Most patients see 50β70% improvement after 3β4 sessions spaced 4β6 weeks apart.
3 Fractional CO2 Laser
Fractional CO2 creates microscopic channels in the skin, triggering aggressive collagen renewal and resurfacing. It is particularly effective for boxcar and rolling scars. It requires 5β7 days of downtime but delivers dramatic results β often 60β80% improvement in a single session. We use it selectively, adjusting settings for Indian skin to minimise pigmentation risk.
4 Chemical Peels
Medium-depth chemical peels (TCA cross technique for ice pick scars, glycolic or salicylic peels for PIH and superficial scars) are an excellent adjunct to other treatments. They are often combined with MNRF or laser for enhanced results.
5 Subcision
For deep rolling scars, subcision is a minor procedure where a needle is used to break the fibrous bands tethering the scar to underlying tissue β allowing the skin surface to rise. It is often combined with fillers or MNRF for optimal results.
6 Realistic Expectations
No treatment offers 100% removal of acne scars β but significant, visible improvement is achievable for almost every patient. A combination protocol tailored to your scar types, skin tone, and lifestyle will give the best results. Most patients achieve 60β80% improvement over a complete treatment course. The earlier treatment begins (ideally once acne is controlled), the better the outcome.
